Recently, Local 79 members received a mailing from the National Life Insurance Company. This company provides all Local 79 members and retirees with a $4,000 Accidental Death and Dismemberment benefit at no charge to them or to Local 79. As a member of Local 79, you are automatically enrolled for coverage and do not need to fill out anything to be eligible to receive this benefit. If you die from an accident and you have not designated a beneficiary with the company, your estate will receive the money. However, if you wish to designate a beneficiary and receive a certificate of coverage, you will need to fill out the card that was sent to you and send it back in the envelope provided by the company.If you did not receive the card or the envelope and wish to, you can call the company at 1-800-503-1426. If you send the card back or call the company, a representative of the company will call on you to arrange to personally deliver the certificate, witness your beneficiary designation, and tell you about other insurance programs available to you.
